  • Launched 17 January 1942
  • Commissioned USS PC-640, 12 July 1942
  • Reclassified SC-640 in April 1943
  • Visited Auckland, New Zealand 7 - 18 October 1944
  • Struck from the Naval Register 21 January 1946
  • Transferred to the Maritime Commission 16 December 1946
  • Fate unknown.
